JOB ANNOUNCEMENT

POSITION:                      Community Health Worker

BASIC FUNCTION:
Meets with residents struggling to meet their basic needs in the community, at their home, and in other community settings to assess social determinants of health (SDOH), increase access to care and supportive services, and offer appropriate referrals to other community supports. The position also requires regular interaction/advocacy with local agencies and providers, routinely updating resource information, and identifying emerging community needs for the Jonnycake Center. Community Health Workers also meet regularly with Jonnycake tenants living in the agency’s service-enriched housing.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Build trusting relationships and serve as an advocate and peer mentor with the goal of empowering residents, individually or in groups, to overcome challenges to good health and well-being.
  • Facilitate appointments, services, and other activities through quality referrals, side-by-side coaching, and transportation assistance.
  • Engage service-enriched housing tenants in goal setting and long-term planning with an emphasis on youth success and opportunities.
  • Works with supervisor, colleagues, and board subcommittees to organize, evaluate, and recommend adjustments to agency programs to respond to community needs.
  • Establishes relationships with health care providers, school social workers, housing authorities, and other local organizations to meet JC member needs.
  • Attends human services and policy meetings at local and state level whenever appropriate to provide awareness of, outreach to, and advocacy for Jonnycake members.
  • Empower residents to become advocates and helpers in their community.
  • Able to relate to residents and build trusting relationships.

LICENSES, TOOLS, AND EQUIPMENT:

  • Valid driver’s license

ENVIRONMENTAL CONDITIONS:

This position is not substantially exposed to adverse environmental conditions.

QUALIFICATIONS:

  • High school diploma/GED.
  • Current Community Health Worker Certification or potential to become certified within six months.
  • Strong ability to engage diverse groups and individuals in a professional, culturally responsive manner.
  • Highly organized with strong interpersonal skills; works well independently and in a team-centered environment.
  • Reliable personal transportation with proof of RI minimum requirements of auto insurance.
  • Demonstrates proficiency with progress notes, record keeping, human services software applications, Microsoft programs, and other technology.
  • Ability to work evenings and weekends as needed.
  • Familiarity with Washington County community and resources; knowledge of culture and values in the local community preferred.
  • Must submit to a criminal background check.

Salary:
Salary commensurate with experience.
